Request an ID Card

Your ID card provides your username for using the Student Portal, Blackboard, and SMU Gmail. The ID card also contains your 14-digit library barcode number, which you can use to check out materials from the Libraries and from affiliated libraries within the MnPALS consortium. If you need a new card, you may request one using the form below.
